At our practice, we prioritize punctuality and respect for everyone's time, ensuring that appointments proceed as scheduled. To assist you, we will provide reminders for your upcoming appointments through phone calls, text messages, or emails.
We kindly request that you arrive 15 minutes before your scheduled time to facilitate a smooth process. We recognize that unforeseen circumstances can occur. Arriving late may necessitate rescheduling your appointment to maintain the schedule for other patients.
Missed Appointments:
Missed appointments represent a missed opportunity to advance care, a restriction on access to care to others, and a missed opportunity to provide professional services for the practice. The practice will assess a fee of $50.00 per missed or cancelled appointment without at least 48 hours’ notice. A fee will be charged for a second missed appointment. A third missed appointment will result in discharge from the practice. For new patients, a fee may be charged if the first appointment is missed and must be paid prior to rescheduling to establish care. We appreciate your understanding and cooperation in these policies, which are designed to optimize care and service for all our patients.
Referrals: Patients are responsible for obtaining and presenting a referral at the time of service, if one is required. If a patient does not obtain a necessary referral prior to the performance of the services, the patient may be responsible for payment in full.
Communication and Behavior:
Our practice is committed to fostering a respectful, supportive, and professional environment for both patients and staff. To uphold these standards, we require all patients to engage in respectful communication and behavior at all times while interacting with our staff and within our facility. As such, our practice has a zero-tolerance policy for any form of disrespectful or abusive behavior. This includes, but is not limited to, verbal abuse, harassment, intimidation, or any other form of conduct that undermines the dignity or safety of our staff or patients. Such behavior will not be tolerated under any circumstances.
Should a patient engage in behavior that is deemed disrespectful or abusive, the clinic will terminate the provision of medical services to that individual. In the event of service termination due to abusive behavior, the patient will be notified and provided with information on alternative healthcare providers, as appropriate. The clinic remains committed to ensuring continuity of care and will assist with the transfer of medical records to another provider if requested.
By receiving care in our practice, patients acknowledge their understanding and agreement to adhere to these standards of behavior, recognizing that failure to comply will result in the discontinuation of services at our practice.
